Our office is located in a new medical office park, Deer Creek Commons, at 129th Street and Metcalf Avenue. From Metcalf Avenue, go west on 129th Street. Take your immediate right turn into the office park. We are the second building on the left.

Insurance Plans

Most insurance plans are accepted. Potential patients can check with their insurance carrier or contact our office. If we are not a contracted provider for your plan, please check your insurance coverage. Many plans provide provisions for you to go outside your plan for care.

It is important to note that you need to bring a referral form from your primary care physician with you if this is required by your insurance carrier.
